Every active ingredient lists its own amount on the label.

Why this rating?
  • The label discloses an exact amount for 1 of its 1 active ingredient.
  • No proprietary blends here — you can verify the dose of every single component.

This product contains one active ingredient: a cascara sagrada bark extract. Cascara sagrada is a stimulant laxative — it works by triggering bowel contractions to move things along.

The liquid is prepared in water and cane alcohol.

Evidence for Intended Use · database check
By FDA rules, dietary supplements can’t claim to treat, cure, or prevent disease — so labels speak in careful marketing language. We discern each product’s intended use from its name, label claims, and label statements, then grade the clinical evidence for that use. How these ratings are computed
Not established

The graded evidence we hold for these ingredients covers different conditions than the ones this product is marketed for, so there's no established rating for its stated use — see Cascara Sagrada's own graded uses on its monograph.

Why this rating?
  • The label markets this product for: Gastrointestinal system restoration.
  • We looked for evidence on: Constipation, Bowel preparation, Bowel regularity, Digestive health.
  • The closest evidence on file: Cascara Sagrada is rated "Possibly Ineffective" for Bowel preparation (Natural Medicines).
  • Also on file: Cascara Sagrada is rated "Insufficient Reliable Evidence To Rate" for Constipation.

The evidence for cascara sagrada is limited. It's rated possibly ineffective for bowel preparation before procedures.

For general constipation, there isn't enough reliable evidence to say how well it works.

How safe is it?

Well-documented data
Safety Information · database check
Well characterized

Adverse-effect, pregnancy, and general safety data are on file for most of these ingredients.

Why this rating?
  • We hold adverse-effect (side-effect) data for 1 of the 1 matched ingredient.
  • Pregnancy & breastfeeding safety ratings cover 1 of 1.
  • General safety write-ups exist for 1 of 1.
  • Remember: this measures how much safety information exists. Thin data is not the same as being safe.

Cascara sagrada is generally well tolerated when you use it short-term and as directed — the most common side effects are mild abdominal cramps and discomfort. But long-term use is where the real concerns come in: it can lead to albuminuria and blood in your urine, and over time may cause severe weight loss (cachexia).

Freshly harvested or improperly aged cascara bark can trigger severe vomiting. The safety data advises against using stimulant laxatives like cascara during pregnancy.

Because active compounds may pass into breast milk, it's best avoided while nursing.

  • The most serious interaction on file is rated Moderate.
  • Some involve high-stakes drug classes: anticoagulant / antiplatelet drugs; heart-rhythm medications.
  • For scale: 746 individual medications appear in the full list. A big number alone doesn't make a product dangerous — what matters is whether YOUR medication is on it, so run yours through the interaction checker on this page.

Check before you start if you take diuretics (water pills), corticosteroids, digoxin, warfarin, other stimulant laxatives, or any medication broken down by your liver's CYP3A4 enzyme. These moderate and minor interactions may affect how your drugs work or raise your risk of low potassium or bleeding.

Cascara Sagrada by Herb Pharm: Common Questions

Does Cascara Sagrada by Herb Pharm interact with any medications?
Yes. Based on its ingredients, Cascara Sagrada has a known interaction with 745 medications. Use the checker to see how it interacts with a specific drug.
How can one product interact with so many drugs?
Cascara Sagrada contains a single active ingredient, and that one ingredient can interact with many different medications on its own. We check it against each medication and show the mechanism responsible.
Where does this information come from?
The product label data comes from the NIH Dietary Supplement Label Database (DSLD); the interaction data is built on the Natural Medicines database and reviewed by HelloPharmacist pharmacists.
Can I use cascara sagrada long-term?
The facts don't establish that cascara works well enough for ongoing use, and long-term use brings real risks — including albuminuria, blood in your urine, and severe weight loss. It's best thought of as short-term help. Talk to your pharmacist about safer long-term options.
What are the most common side effects?
Mild abdominal discomfort and cramps are the most common. Serious side effects are rare with proper use, but fresh or improperly aged cascara bark can cause severe vomiting.
Is cascara sagrada safe during pregnancy?
No. The safety data advises against stimulant laxatives like cascara during pregnancy. Talk with your doctor or pharmacist about safer alternatives.
Can I take cascara sagrada while breastfeeding?
The active compounds may pass into breast milk, so it's best avoided while nursing. Check with your pharmacist about safer options.
Why do I need to check my medications before taking this?
Cascara can lower your potassium level — a risk that gets worse if you also take diuretics, corticosteroids, or digoxin. It can also thin your blood indirectly if it causes diarrhea, raising bleeding risk with warfarin. These interactions are serious enough that you need to run your exact medications through the checker on this page.
