Metabolic Lean contains four active ingredients. The Meratrim Proprietary Blend (a combination product) is paired with mangosteen fruit rind extract, chromium, and Sphaerantus indicus flower heads extract, plus capsaicinoids — compounds found in hot peppers.
The product also includes inactive ingredients (capsule material, binders, and flow agents) to hold everything together.
Does it work?
Strong evidence
Mangosteen extract is possibly effective for periodontitis (gum infection), but evidence is insufficient to rate it for hair loss, Alzheimer's disease, or eczema. Chromium is likely effective for chromium deficiency and possibly effective for type 2 diabetes.
Evidence rates chromium as possibly ineffective for prediabetes, schizophrenia, and high blood pressure. We don't have effectiveness data on file for the Meratrim blend, Sphaerantus indicus, or capsaicinoids in this product.
How safe is it?
Well-documented data
Chromium is generally well tolerated at typical supplement doses; long-term or high doses may carry risks. The most common side effects are gastrointestinal irritation, headaches, insomnia, irritability, and mood changes.
Rare cases of kidney or liver damage and low platelet counts have been reported. Chromium can also trigger skin rashes or contact dermatitis in sensitive people.
Mangosteen is generally considered safe as a food, but long-term safety of concentrated supplements isn't well studied. At doses up to 560 mg daily, it's been associated with mild tiredness, headache, dizziness, nausea, vomiting, constipation, diarrhea, dry throat, and cough — though it's unclear if these were actually caused by mangosteen.
During pregnancy, the safety data advises against mangosteen supplements; the same applies to breastfeeding. Chromium in small amounts from food or a prenatal vitamin is generally considered acceptable, but extra supplement doses should be avoided in pregnancy and breastfeeding unless your doctor recommends it.

Chromium
Antidiabetes Drugs
Theoretically, chromium may have additive effects with antidiabetic agents and increase the risk of hypoglycemia.
Some research shows that taking chromium might lower blood glucose levels, especially in patients with poorly controlled type 2 diabetes.
Insulin
Theoretically, concomitant use of chromium and insulin might increase the risk of hypoglycemia.
In clinical research, chromium has been shown to increase insulin sensitivity,
Levothyroxine (Synthroid, Others)
Chromium might bind levothyroxine in the intestinal tract and decrease levothyroxine absorption.
Clinical research in healthy volunteers shows that taking chromium picolinate 1000 mcg with levothyroxine 1 mg decreases serum levels of levothyroxine by 17% when compared to taking levothyroxine alone. Advise patients to take levothyroxine at least 30 minutes before or 3-4 hours after taking chromium.
Aspirin
Theoretically, aspirin might increase chromium absorption.
Animal research suggests that aspirin may increase chromium absorption and chromium levels in the blood.
Nonsteroidal Anti-Inflammatory Drugs (Nsaids)
NSAIDs might increase chromium levels in the body.
Drugs that are prostaglandin inhibitors, such as NSAIDs, seem to increase chromium absorption and retention.
Garcinia mangostana [fruit rind] extract
Anticoagulant/Antiplatelet Drugs
Theoretically, concomitant use of mangosteen with anticoagulant or antiplatelet drugs may increase the risk of bleeding.
In vitro and animal research shows that gamma-mangostin, a constituent of mangosteen, is a potent and competitive antagonist of the serotonin 2A (5-HT2A) receptor. Antagonism of the 5-HT2A receptor is believed to reduce platelet aggregation.
Donepezil (Aricept)
Theoretically, concomitant use of mangosteen with donepezil might increase the effects of donepezil.
Animal research shows that concomitant use of an aqueous extract of mangosteen pericarp with donepezil increases brain concentrations of donepezil at 4 hours by 64% without associated effects on systemic exposure.
