Chromium and Nepafenac

Here is the interaction we found between these two, with the evidence behind it.

Minor

NONSTEROIDAL ANTI-INFLAMMATORY DRUGS (NSAIDs)

Why this applies to Nepafenac

Nepafenac falls under NONSTEROIDAL ANTI-INFLAMMATORY DRUGS (NSAIDs) — the group Chromium interacts with.

NSAIDs might increase chromium levels in the body.
Drugs that are prostaglandin inhibitors, such as NSAIDs, seem to increase chromium absorption and retention.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something to keep in perspective about pairing chromium with nepafenac (Nevanac or Ilevro). The note we have flips the usual script: it isn't that nepafenac becomes stronger, but rather that NSAIDs may nudge your body to absorb and hold onto a bit more chromium. So this is really about your chromium levels, not your eye drops.

The good news is that nepafenac is an eye drop used after cataract surgery, so very little of it reaches the rest of your body. That makes any real-world effect on chromium quite unlikely. On top of that, this interaction is theoretical, based on how these drugs work rather than actual reports. I wouldn't worry, but feel free to mention your chromium supplement to your pharmacist or doctor.

Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Nepafenac page

“Nepafenac (Nevanac, Ilevro) is an anti-inflammatory eye drop given after cataract surgery, and enough of it is absorbed to act like an NSAID throughout the body. Its interaction list is short, nine ingredients in all, but it warrants real vigilance because the two rated major both involve alcohol.”

About Chromium

What is Chromium?

Chromium
  • Blood sugar support in type 2 diabetes
  • Weight loss and appetite control
  • Insulin sensitivity
  • Cholesterol and lipid support
  • General nutritional supplement

Chromium is an essential trace mineral involved in how the body handles sugar and fat. Some studies suggest it may modestly help blood sugar control in certain people with type 2 diabetes, but results are mixed and the effect on weight loss is small and uncertain. For most healthy people who eat a varied diet, supplements are not needed; check with your pharmacist or doctor before starting.
