How safe is it?

Peppermint leaf and peppermint tea in everyday amounts are generally well tolerated. The concentrated oil — which is what's in this liquid supplement — should be used carefully and only with your provider's approval.
Most side effects from taking peppermint orally are mild: abdominal pain, heartburn, belching, dry mouth, diarrhea, or nausea. Topically, it can cause burning or skin irritation.
Rare allergic reactions (including anaphylaxis) have been reported. In large amounts, undiluted peppermint oil may cause burns in the mouth or throat.

Peppermint Liquid does interact with a number of medications — specifically through its peppermint content, which can affect how your liver processes certain drugs. The most serious concern involves a group of medications broken down by an enzyme called CYP3A4 (cytochrome P450 3A4).
Clinical research shows that peppermint oil can slow this enzyme down, raising drug levels in your bloodstream. Felodipine, a blood pressure medication, is the best-documented example — a single 600 mg dose of peppermint oil increased its levels in healthy volunteers.

Peppermint may also theoretically raise levels of drugs processed by two other liver enzymes: CYP2C19 and CYP2C9 — though these interactions haven't been confirmed in humans yet, only in lab studies. Cyclosporine, an immunosuppressant (a drug that tamps down the immune system), is another concern; animal research suggests peppermint oil may boost its levels, though this too hasn't been seen in people.
A smaller concern exists with CYP1A2 substrates — drugs like certain antidepressants or stimulants — though peppermint tea had minimal effect on caffeine, a CYP1A2 drug, in a human trial.

Cyclosporine (Neoral, Sandimmune)
Theoretically, peppermint oil might increase the levels and adverse effects of cyclosporine.
In animal research, peppermint oil inhibits cyclosporine metabolism and increases cyclosporine levels. Inhibition of cytochrome P450 3A4 (CYP3A4) may be partially responsible for this interaction. An interaction between peppermint oil and cyclosporine has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, peppermint might increase the levels of CYP2C19 substrates.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, peppermint might increase the levels of CYP2C9 substrates.
In vitro research shows that peppermint oil inhibits CYP2C9. So far, this interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, peppermint might increase the levels of CYP3A4 substrates.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.
