Peppermint Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Peppermint against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

HelloPharmacist Scorecard of Peppermint by Tonic Tinctures
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This product contains one active ingredient: peppermint leaf tincture, a concentrated liquid extract of peppermint. The inactive ingredients are water, alcohol, and stevia leaf extract.
Does it work?
Strong evidence
Peppermint is likely effective for irritable bowel syndrome (IBS). It's possibly effective for dyspepsia (indigestion), chemotherapy-induced nausea and vomiting, barium enema-related colon spasm, and endoscopy-related side effects.
How safe is it?
Well-documented data
Peppermint leaf and tea in food amounts are generally well tolerated. Concentrated peppermint oil should be used carefully and only with your provider's approval.
The most common side effects from oral use are abdominal pain, anal burning, belching, diarrhea, dry mouth, heartburn, nausea, and vomiting. Rarely, allergic reactions have been reported, including one case of anaphylaxis (severe swelling of the lips, tongue, and throat).
Topically, peppermint oil can cause burning, skin irritation, and contact dermatitis; undiluted oil may cause chemical burns. Peppermint is rated likely safe in pregnancy and lactation.
Meds to double-check
Moderate interaction found
If you take any blood pressure medications, immunosuppressants like cyclosporine, or other drugs processed through the liver enzymes CYP3A4, CYP2C19, or CYP2C9, check your specific medications with the tool below before starting this product. Most interactions are theoretical and haven't been seen in humans, but the CYP3A4 effect on felodipine was proven in clinical research.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Peppermint tincture is a solid choice if you're dealing with IBS or indigestion and have no medications that interact with liver enzymes. If you take drugs metabolized by CYP3A4, CYP2C19, or CYP2C9, or if you use cyclosporine, check with your pharmacist first.
Talk to your provider before using concentrated peppermint oil if you're pregnant, breastfeeding, or have a history of allergies.

Peppermint by Tonic Tinctures Drug Interactions
HelloPharmacist Interaction Report
Tonic Tinctures Peppermint Leaf Tincture does interact with a number of medications.
The most serious documented interaction involves drugs that depend on the liver enzyme CYP3A4 (CYP3A4 substrates) — a single dose of peppermint oil has been shown in clinical research to slow how your liver breaks down these drugs, which can raise their levels in your blood. Felodipine, a blood pressure medication, is the example studied.
This is a Moderate severity interaction.
Read the full breakdown — every affected drug type, severity by severity
Peppermint also theoretically affects two other liver enzymes: CYP2C19 and CYP2C9, both at Moderate severity. Lab research shows peppermint oil inhibits these enzymes, but no interactions have been reported in humans so far.
Additionally, there's a Moderate interaction with cyclosporine (an immunosuppressant used after transplant), based on animal research showing peppermint oil raises cyclosporine levels. Finally, a Minor theoretical interaction exists with CYP1A2 substrates, though clinical research with caffeine (a CYP1A2 substrate in coffee and tea) found no significant effect.
No interactions are documented in our data for peppermint leaf itself when used as tea or dilute extract at typical doses, but the concentrated oil in supplements is where the interactions emerge. Use the medication checker below to see if any of your specific drugs are affected.

For each ingredient in Peppermint with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Peppermint Leaf Tincture
Cyclosporine (Neoral, Sandimmune)
Theoretically, peppermint oil might increase the levels and adverse effects of cyclosporine.
In animal research, peppermint oil inhibits cyclosporine metabolism and increases cyclosporine levels. Inhibition of cytochrome P450 3A4 (CYP3A4) may be partially responsible for this interaction. An interaction between peppermint oil and cyclosporine has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, peppermint might increase the levels of CYP2C19 substrates.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, peppermint might increase the levels of CYP2C9 substrates.
In vitro research shows that peppermint oil inhibits CYP2C9. So far, this interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, peppermint might increase the levels of CYP3A4 substrates.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.
