The Lactobacillus strains include salivarius, plantarum, paracasei, acidophilus, casei, rhamnosus, bulgaricus, brevis, gasseri, and fermentum. The Bifidobacterium strains include longum infantis, bifidum, longum, breve, and animalis lactis (listed twice).
These live organisms are designed to reach your intestines in viable form — the "D.R." likely refers to delayed release, meaning the capsule resists stomach acid so the bacteria survive to where they're meant to work. The inactive ingredients include maltodextrin, hypromellose (a capsule component), pectin, water, medium chain triglyceride, and silica — all standard excipients to form and stabilize the capsule.

The evidence picture is mixed. Lactobacillus acidophilus is possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections. Bacillus subtilis DE111 is possibly effective for antibiotic-associated diarrhea.
Inulin is possibly effective for obesity, constipation, and diabetes. For most other uses claimed on probiotic labels — like age-related cognitive decline, hay fever, anxiety, asthma, and athletic performance — the evidence we hold is insufficient to rate them.
A few ingredients (Bifidobacterium breve for cognitive decline and sepsis, Lactobacillus acidophilus for atopic disease) were rated possibly ineffective in the studies reviewed.
How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy adults and children, but they come with important caveats. Anyone who is seriously ill, has a weakened immune system, or is critically ill should check with their doctor first — there are rare cases of the bacteria themselves causing serious bloodstream infections (bacteremia or sepsis) in these high-risk groups, particularly in preterm infants with critical illness.
Gastrointestinal side effects are uncommon but can happen: bloating, flatulence, abdominal discomfort, constipation, or diarrhea have been reported rarely with various strains. Inulin in particular commonly causes gas and bloating, especially at higher doses, so your gut may need time to adjust.
For pregnancy and breastfeeding, Bifidobacterium longum and inulin are rated possibly safe. Lactobacillus acidophilus and Bifidobacterium bifidum are rated possibly safe in pregnancy; lactation data are not on file.

Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Bifidobacterium bifidum
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Lactobacillus gasseri
Antibiotic Drugs
Theoretically, taking Lactobacillus gasseri with antibiotic drugs might decrease the effectiveness of L. gasseri.
Lactobacillus gasseri preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. gasseri preparations by at least two hours.

Inulin
Antidiabetes Drugs
Theoretically, inulin might increase the risk of hypoglycemia with antidiabetes drugs.
Some clinical research shows that inulin improves glycemic control in patients with diabetes; however, it is unclear if it has hypoglycemic effects.
