Guggul is rated possibly ineffective for obesity and has insufficient evidence for acne, hemorrhoids, and high cholesterol. Garcinia also has insufficient evidence for athletic performance, nonalcoholic fatty liver disease, high cholesterol, and obesity.

How safe is it?
Well-documented data
Myrrh is generally well tolerated in short-term topical or mouth-rinse use, but oral safety when taken by mouth hasn't been well studied. Gastrointestinal side effects include diarrhea at doses of 2–4 grams.
Guggul is generally well tolerated orally, though digestive upset (belching, bloating, diarrhea, nausea, vomiting) and skin reactions (rash, itching, swelling) are the most common adverse effects; skin reactions appear dose-dependent, with higher doses carrying greater risk. Rarely, guggul has been linked to rhabdomyolysis (muscle breakdown).
Garcinia is generally well tolerated short-term, with common side effects being diarrhea, gastrointestinal discomfort, headache, and nausea. Rare serious effects include liver injury, mania, and in one case, a heart inflammation that progressed to heart failure.
All three ingredients advise against use in pregnancy because of uterine-stimulating effects (myrrh and guggul) or insufficient safety data (garcinia), and lactation safety data are not on file for any of them.

The product interacts with multiple medication types, and the most serious documented concern involves myrrh (in Shuddha Guggulu), which may theoretically increase the risk of low blood sugar (hypoglycemia) when combined with antidiabetes drugs — a Moderate-severity interaction based on animal research suggesting myrrh can lower blood glucose.

Guggul (in Trimad Churna) has the broadest range of interactions. It may reduce the effectiveness of the heart medication propranolol and the calcium-channel blocker diltiazem; it could theoretically reduce the effects of many other medications processed by the liver enzyme CYP3A4; it may increase bleeding risk with blood thinners (anticoagulants) or antiplatelet drugs; it might interfere with hormonal contraceptives and estrogen therapy by acting as an estrogen receptor agonist; and it could interfere with thyroid hormone therapy or the breast cancer drug tamoxifen through similar hormonal effects.
All of these are Moderate-severity interactions.
Myrrh also has a Moderate-severity interaction with warfarin (Coumadin), a blood thinner — one case report documents a patient's warfarin effectiveness declining after taking myrrh. Garcinia (in Vrikshamla) may increase serotonin-related side effects with antidepressants or other serotonergic drugs, increase bleeding risk with blood thinners, theoretically worsen low blood sugar with antidiabetes drugs, and in rare cases is linked to liver damage—especially concerning if you take other potentially hepatotoxic medications.

Trimad Churna
Anticoagulant/Antiplatelet Drugs
Theoretically, guggul might increase the risk of bleeding when taken with anticoagulant/antiplatelet drugs.
In vitro research and preliminary clinical studies suggest that guggul might have antiplatelet and anticoagulant effects.
Contraceptive Drugs
Theoretically, guggul might increase the risk of adverse effects when taken with contraceptive drugs.
In vitro research shows that guggul has estrogen-alpha receptor agonist activity.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.
Diltiazem (Cardizem, Others)
Guggul might reduce the effects of diltiazem.
A small pharmacokinetic study shows that concomitant use of guggul with diltiazem reduces the bioavailability of diltiazem.
Estrogens
Theoretically, guggul might increase the risk of adverse effects when taken with estrogens.
In vitro research shows that guggul constituents known as guggulsterones have estrogen-alpha receptor agonist activity.
Propranolol (Inderal)
Guggul might reduce the effects of propranolol.
A small pharmacokinetic study shows that concomitant use of guggul with propranolol reduces the bioavailability of propranolol.
Rosuvastatin (Crestor)
Theoretically, guggul might increase the effects and adverse effects of rosuvastatin.
Animal research shows that guggul increases the bioavailability and hypolipidemic effects of rosuvastatin. The mechanism of this interaction is unclear.
Tamoxifen (Nolvadex)
Theoretically, guggul might interfere with tamoxifen therapy.
In vitro research shows that guggul has estrogen-alpha receptor agonist activity.
Thyroid Hormone
Theoretically, guggul might increase the risk for adverse effects when taken with thyroid hormone therapy.
Animal research suggests that guggul has thyroid-stimulating effects.
Shuddha Guggulu
Antidiabetes Drugs
Theoretically, myrrh might increase the risk of hypoglycemia when taken with antidiabetes drugs.
In vitro and animal research suggests that myrrh has hypoglycemic effects.
Warfarin (Coumadin)
Theoretically, myrrh might decrease the effectiveness of warfarin.
In one case, a patient who was previously stable on warfarin had a significant decline in international normalized ratio (INR) following consumption of an aqueous extract of myrrh.
