Ultra Pro-20 contains 21 ingredients, of which 15 are live probiotic strains — bacteria and one yeast — that are intended to support digestive and immune health. The checked strains include several Lactobacillus species (L. acidophilus, L. gasseri, L. bulgaricus, L. helveticus), multiple Bifidobacterium species (B. breve, B. bifidum, B. longum), Streptococcus thermophilus, Bacillus subtilis, and Saccharomyces boulardii, a beneficial yeast.
The product also contains prebiotic fructooligosaccharides (a carbohydrate that feeds beneficial bacteria) and six inactive ingredients: purified water, stevia, natural flavor, potassium sorbate, and citric acid.
Does it work?
Moderate evidence
The evidence for most of these strains is still emerging. L. acidophilus and B. bifidum show possibly effective evidence for irritable bowel syndrome (IBS), and L. acidophilus is possibly effective for antibiotic-associated diarrhea and bacterial vaginosis.
B. longum has insufficient evidence for the conditions listed in our data. S. boulardii stands out with strong possibly effective ratings for travelers' diarrhea, antibiotic-associated diarrhea, and rotaviral diarrhea.
For many other uses — allergic rhinitis, anxiety, asthma, cognitive function — the evidence we hold rates as insufficient. The product has not been studied as a whole, so effectiveness depends on the individual strains and the dose.
How safe is it?
Well-documented data
Most of these probiotic strains are generally well tolerated in healthy adults and children. However, live microorganisms do carry a small risk: rare cases of bacterial or fungal bloodstream infections (bacteremia, sepsis) have been reported, especially in critically ill patients, those with weakened immune systems, or people with central lines or catheters.
Gastrointestinal side effects — bloating, flatulence, abdominal discomfort, and diarrhea — are uncommon but can occur. One trial reported a rash with itching in a small number of participants, though it's unclear whether the probiotics were responsible.
We hold no pregnancy or breastfeeding data for most of these strains; B. longum is rated possibly safe in pregnancy. Because safety data are limited, talk with your doctor before use if you are very ill, immunocompromised, or pregnant.

Separate antibiotic drugs from Ultra Pro-20 by at least 2–3 hours — antibiotics will kill the beneficial bacteria in the product and reduce its effectiveness. Antifungal medications can similarly reduce the viability of the S. boulardii yeast in this product.

Several of the live bacterial strains in this product, including L. acidophilus, B. breve, B. bifidum, B. longum, L. gasseri, L. bulgaricus, and B. subtilis, may lose effectiveness if you take them at the same time as antibiotics. Antibiotics can kill the beneficial organisms in the product.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
B. breve
Antibiotic Drugs
Theoretically, taking Bifidobacterium breve with antibiotic drugs might decrease the effectiveness of B. breve.
Since B. breve preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. breve preparations by at least 2 hours.
B. bifidum

Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
B. longum

L. helveticus

Theoretically, taking Lactobacillus helveticus with antibiotic drugs might decrease the effectiveness of L. helveticus.
Lactobacillus helveticus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. helveticus preparations by at least two hours.
B. subtilis
Antibiotic Drugs
Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
S. cerevisiae
Monoamine Oxidase Inhibitors (Maois)
Theoretically, taking brewer's yeast with MAOIs might increase the risk of hypertension.
Brewer's yeast contains tyramine. Taking brewer's yeast with MAOIs might increase the risk for hypertensive crisis.
Antidiabetes Drugs
Taking brewer's yeast with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking chromium-containing brewer's yeast can decrease levels of blood glucose in diabetic patients being treated with antidiabetes drugs.
Lithium
Theoretically, taking brewer's yeast with lithium might cause additive effects and side effects.
Some brewer's yeast products contains lithium.
Antifungals
Theoretically, taking antifungals with some brewer's yeast products might decrease the effectiveness of brewer's yeast.
Some brewer's yeast products contain live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
S. boulardii

Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
