Why this applies to Repotrectinib

Repotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the short version for anyone on Augtyro (repotrectinib), the targeted cancer medicine for certain lung and solid tumors. Your body clears repotrectinib mainly through a liver enzyme called CYP3A4. Aegeline has been shown, in test-tube studies only, to slow that same enzyme down. In theory, that could let repotrectinib build up higher than intended, which might mean more of its typical side effects like dizziness, trouble with balance or coordination, numbness or tingling, fatigue, and changes in taste.

Keep this in perspective though. This is a weak, lab-only signal, not something reported in real patients. The bigger issue is aegeline itself, since it has been tied to serious liver injury and regulators have warned against it. I'd steer clear of it entirely and check with your oncology team before adding any supplement.
