American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with Uniretic, which combines hydrochlorothiazide (a water pill) with moexipril (an ACE inhibitor). The hydrochlorothiazide piece can flush out potassium and magnesium through your urine. When those minerals run low, your heart becomes more prone to a dangerous rhythm change called QT prolongation. American hellebore is a true poison that can do its own damage to heart rhythm, heart rate, and blood pressure, so stacking the two could raise the odds of fainting, palpitations, or a serious irregular heartbeat.

This is rated a major interaction, though the evidence is fairly limited. Honestly, American hellebore is not something I'd recommend taking at all because of its toxicity. Please talk with your pharmacist or doctor before going near any product containing it.

American hellebore can have potent cardiovascular effects due to the toxic steroid ester alkaloids it contains. The alkaloids in this plant notably reduce blood pressure even in small doses. Additionally, the alkaloids can exhibit cardiac depressant, bradycardic, and sedative effects. These alkaloids inhibit the inactivation of sodium-ion channels in excitable cells, particularly those regulating cardiac activity. Furthermore, American hellebore's alkaloids induce vasodilation and block beta-adrenergic activity, potentially influencing the effectiveness or side effects of antihypertensive medications (source https://pubmed.ncbi.nlm.nih.gov/5457326/). Patients should exercise caution and consult healthcare professionals before using American hellebore for any reason.

Here's the thing to know about Uniretic: it combines hydrochlorothiazide (a water pill) with moexipril (an ACE inhibitor), and both of those already work to lower your blood pressure. American hellebore is a different animal. This plant is genuinely toxic, and even tiny amounts can drop your blood pressure and slow your heart rate sharply on their own.

Put the two together and those blood-pressure-lowering effects stack up. You could end up feeling dizzy or lightheaded, faint when you stand up, or notice an unusually slow heartbeat. That's a dangerous combination, not a mild one. I'd steer clear of any non-homeopathic hellebore product entirely, and please check with your pharmacist or doctor before going anywhere near it.
