Risperidone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the gist with these two together. Lab studies hint that ashwagandha may speed up an enzyme called CYP3A4, which is one of the pathways your body uses to clear risperidone. If that happened in real life, your body could break risperidone down a little faster, possibly making it work less well. For someone using it for schizophrenia or bipolar symptoms, that could mean a return of issues like mood swings, agitation, or unusual thinking.

That said, I don't want to overstate this. This concern comes only from test-tube research, not from people, and it's rated as a minor, theoretical interaction. Risperidone is not a prodrug, so the math is straightforward. Before adding ashwagandha, just run it by your pharmacist or prescriber so they can keep an eye on how you're doing.

Risperidone also falls under Serotonergic Drugs.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the short version: this one is mostly theoretical. Risperidone nudges serotonin activity in the brain, and some animal studies hint that ashwagandha may do the same by tweaking certain serotonin receptors. On paper, two things that both touch serotonin could add up. But in real life there are no human case reports of problems from this combo, and the evidence behind it is weak.

So I wouldn't lose sleep over it, but it's worth a quick heads-up to your pharmacist or doctor. If you did notice things like unusual restlessness, agitation, shakiness, a racing heart, sweating, or feeling overly drowsy or confused, mention it. Don't start or stop either one on your own, just loop in your care team first.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. What Is Risperidone Used For?

Brand names: Risperdal, Perseris

Risperidone is prescribed for three main conditions: schizophrenia (a disorder affecting how a person thinks, feels, and perceives reality), acute manic or mixed episodes in Bipolar I Disorder, and irritability associated with autistic disorder — including aggression, self-injury, temper tantrums, and rapid mood swings.

For bipolar mania, it can be used alone or alongside lithium or valproate. It is approved for use in both adults and certain age groups of children and adolescents, depending on the condition being treated.
