Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

This combo product has a lot going on, but the piece that matters here is the hydrocodone. Hydrocodone is a prodrug, meaning your liver's CYP2D6 enzyme turns part of it into hydromorphone, which is what gives some of the pain relief. Lab studies suggest the harmine and harmaline in ayahuasca can block CYP2D6. If that block happens, your body may make less of that active form, so the pain control could actually feel weaker, not stronger.

I want to be honest that this is only theory from test-tube work and hasn't been shown in real people. The bigger concern is that ayahuasca acts like a strong MAOI, which can react dangerously with cold and pain medicines. Please don't mix these without talking to your doctor or pharmacist first.

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

This combo product (Hycomine Compound) includes hydrocodone, an opioid that your liver's CYP3A4 enzyme helps process. The lab data here suggests that compounds in ayahuasca may slow down that enzyme. If that happened in the body, hydrocodone could build up a bit more than usual, which might mean stronger drowsiness, dizziness, slowed breathing, or constipation. The caffeine and chlorpheniramine in this product can also be affected by changes in liver processing.

I want to be honest: this is only a theoretical concern from test-tube studies, not something proven in people. That said, ayahuasca also acts as an MAOI, which is a separate and more serious worry with this medicine. Please check with your doctor or pharmacist before mixing the two.
