Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for Sirturo (bedaquiline): your body relies on a liver enzyme called CYP3A4 to clear this medication out. Ayahuasca contains beta-carboline compounds that, at least in lab studies, can block that same enzyme. If that blocking happened in a real person, bedaquiline could build up to higher levels than intended.

Why does that matter? Bedaquiline can affect your heart's rhythm (the QT interval) and stress your liver, so higher levels could mean a greater chance of those problems. That said, this is theoretical and comes only from test-tube work, never confirmed in humans. Ayahuasca also carries its own serious risks. Please don't change anything on your own; check in with your doctor or pharmacist first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
