Fedratinib Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the basic idea with fedratinib (Inrebic). Your liver uses an enzyme called CYP3A4 to break this medicine down and clear it from your body. Ayahuasca contains beta-carboline compounds that, in lab studies, can block that same enzyme. If that blocking happened in a real person, fedratinib could build up higher than expected, which may mean more of its known side effects: nausea, diarrhea, fatigue, low blood counts (anemia, easy bruising), and liver enzyme changes.

I want to be honest, though: this is theoretical, based only on test-tube research, and has not been shown in people. Still, fedratinib is a serious cancer medicine, and ayahuasca carries its own real risks. Please don't change anything on your own. Talk with your oncologist or pharmacist first.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
