Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Your Kisqali plus Femara combo gives you two cancer medicines. Both letrozole and ribociclib are partly cleared by a liver enzyme called CYP3A4, and ribociclib in particular leans heavily on it. The concern here is that ayahuasca contains beta-carboline compounds that, in lab studies, can slow that same enzyme down. If that slowing happened in real life, these drugs could build up higher than intended.

For ribociclib especially, higher levels can mean more nausea, fatigue, low blood counts, and changes to your heart's rhythm (QT prolongation). I want to stress this is theoretical right now, only seen in test tubes, not proven in people. Still, given how serious these drugs are, please don't mix them without talking to your oncologist or pharmacist first.
