Why this applies to Macitentan
Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for Opsumit (macitentan): your body relies on an enzyme called CYP3A4 to break it down. Lab studies hint that the beta-carboline compounds in ayahuasca can slow that same enzyme. If that happened, macitentan could build up in your blood and push its effects a little harder than intended.
For macitentan that might mean more pronounced side effects like a stuffy nose, headache, low blood pressure, swelling, or feeling unusually tired. I want to be honest though: this is only seen in test-tube research, not in real people, so it's theoretical for now. Ayahuasca also carries its own serious risks, so please loop in your doctor or pharmacist before mixing anything with your PAH medicine.

What Is Macitentan Used For?
Brand name: Opsumit
Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.
In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
