Mometasone, formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the thing with Zenhale (mometasone plus formoterol): it's an inhaler you breathe in, so most of the medicine works right in your lungs and very little reaches your bloodstream. The concern on file is that ayahuasca's beta-carboline compounds (harmine and harmaline) may block a liver enzyme called CYP2D6, which could in theory let a bit more drug build up. The formoterol part is the one to watch, since extra amounts can cause a fast or pounding heart, jitteriness, shakiness, or a headache.

I want to be honest though: this is only a lab-based, theoretical interaction that hasn't been shown in real people, so I wouldn't lose sleep over it. The bigger issue is that ayahuasca itself is risky and best avoided. Please loop in your doctor or pharmacist before mixing the two.

Your Zenhale inhaler combines two medicines: mometasone (an inhaled steroid) and formoterol (a long-acting bronchodilator that opens your airways). The mometasone part is cleared by a liver enzyme called CYP3A4. The idea here is that compounds in ayahuasca may slow that enzyme down, which could let a bit more steroid linger in your body than usual.

I want to be honest about how soft this is. This comes from lab dish studies only, and it has never actually been shown in people. With an inhaler, very little reaches your bloodstream to begin with, so any real effect is likely small. Still, ayahuasca carries its own serious risks, so please talk with your doctor or pharmacist before combining anything with it.
