Why this applies to Sertraline

Sertraline falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the concern with mixing Sertraline (Zoloft) and ayahuasca. Sertraline already raises serotonin in your brain, which is how it helps with mood and anxiety. Ayahuasca pushes serotonin up too, partly because its vine compounds act like MAO inhibitors and partly from the DMT itself. Stacking the two could send serotonin too high.

When serotonin climbs too far you can get serotonin syndrome: shaking, sweating, shivering, fast heartbeat, confusion, nausea, or agitation. A real case of this happened with a similar antidepressant plus ayahuasca. This warning is mostly theoretical and based on pharmacology, but the risk is taken seriously because it can become dangerous. Please don't change anything on your own. Talk with your pharmacist or doctor first.

Sertraline also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the thing to understand with sertraline (Zoloft) and ayahuasca. The lab-based concern is that ayahuasca's beta-carboline compounds may slow down CYP3A4, one of the liver enzymes that helps clear sertraline. If that enzyme is blocked, sertraline could build up a bit, which might mean more nausea, jitteriness, trouble sleeping, or diarrhea. But honestly, this is theoretical, seen only in test-tube studies and never confirmed in people, so I wouldn't lose sleep over it.

My bigger worry by far is a separate, well-known danger: ayahuasca acts like an MAO inhibitor, and combining that with sertraline can cause serotonin syndrome, which can be serious. Please talk with your doctor or pharmacist before mixing these.

What Is Sertraline Used For?

Brand name: Zoloft

Sertraline treats a range of mental health conditions. All formulations are approved for major depressive disorder (MDD) in adults and obsessive-compulsive disorder (OCD). The oral solution (including Zoloft oral solution) and several tablet formulations are also approved for panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).

For OCD specifically, the capsule formulations and some tablet products are approved for use in adults and children aged 6 and older. Sertraline hydrochloride tablets (plain, not film-coated) are approved for OCD in pediatric patients as well. However, sertraline is not approved for major depressive disorder in children — only in adults.
