Why this applies to Tapentadol

Tapentadol falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the thing about Tapentadol (Nucynta): besides easing pain like a typical opioid, it also boosts norepinephrine and nudges serotonin activity in your nervous system. Ayahuasca brings its own serotonin-raising punch, partly because the vine acts like an MAO inhibitor and partly from its DMT content. Stack those two together and you can pile up too much serotonin at once.

That raises the risk of serotonin syndrome, which can show up as shivering, sweating, a racing heart, muscle twitching, agitation, confusion, or nausea. It can get serious fast. This warning is theoretical, based on how these compounds behave, not on direct studies with Tapentadol, but the concern is real enough that I'd urge you to talk with your doctor or pharmacist before combining them.

About Tapentadol

What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
