Vandetanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for vandetanib (Caprelsa). Your body partly clears this cancer drug using a liver enzyme called CYP3A4. Lab studies suggest the beta-carboline compounds in ayahuasca can block that enzyme. If they slow it down, vandetanib could build up in your blood and hit harder than intended. That might mean more diarrhea, nausea, rash, high blood pressure, or the heart-rhythm change (QT prolongation) that vandetanib is already known for.

I want to be honest about the strength here: this is theoretical, seen only in test-tube work, never confirmed in real people. But vandetanib is a serious drug with a slim safety margin, and ayahuasca carries its own real risks. Please don't combine them without talking to your oncologist or pharmacist first.

Caprelsa (vandetanib) is used to treat medullary thyroid cancer — a type of thyroid cancer — in adults whose tumor cannot be surgically removed or has spread (locally advanced or metastatic disease) and is causing symptoms or actively growing.

Caprelsa should be used with extra caution in patients whose cancer is slow-growing and not causing symptoms, because the serious risks of the medicine must be carefully weighed against any potential benefit in that setting.
