Here's the gist for Bosentan (Tracleer): this medicine already relaxes blood vessels to ease the high pressure in your lungs, which can also nudge your overall blood pressure down a bit. Baikal skullcap contains a compound called baicalein that, in animal studies only, appeared to lower blood pressure too. So the worry is simply that the two together could add up and drop your pressure more than expected.

If that happened, you might feel dizzy, lightheaded, tired, or faint, especially when standing up quickly. To be clear, this is a theoretical concern based on lab and animal work, not strong human evidence, so it's not a reason to panic. Still, before adding skullcap, please run it by your pharmacist or doctor so they can keep an eye on your blood pressure.

Bosentan also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, Baikal skullcap might alter the levels and clinical effects of OATP substrates.
Some pharmacokinetic research shows that baicalin, a constituent of Baikal skullcap, can decrease plasma levels of rosuvastatin. The mechanism is thought to involve stimulation of the activity of the organic anion-transporting polypeptide 1B1 (OATP1B1), which transports rosuvastatin into the liver. This decreases plasma levels of the drug, but increases levels at the site of action in the liver. The degree to which rosuvastatin levels are affected depends on the OATP1B1 haplotype of the individual. Baikal skullcap might also affect other OATP1B1 substrates.

Here's the gist for Bosentan (Tracleer). Your liver pulls bosentan in using a transporter called OATP1B1. Lab and early research suggest that baicalin, a compound in Baikal skullcap, may actually speed up that transporter. In theory, that could move a little more bosentan into the liver and lower the amount circulating in your bloodstream, which might slightly weaken its effect on your lung blood pressure.

I want to be honest though: this is mostly theoretical, drawn from studies on a different drug (rosuvastatin), and it's rated a minor interaction. Bosentan is too important to leave to guesswork, so since PAH is serious, please check with your pharmacist or doctor before adding skullcap. Don't change your bosentan dose on your own.

What Is Bosentan Used For?

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
