Here's the deal with Prempro (your conjugated estrogens plus medroxyprogesterone) and Baikal skullcap. This herb seems to act a little like estrogen itself, and in lab dishes its compounds (like baicalein) can latch onto the same estrogen receptors your hormone therapy is meant to work on. In theory, large supplement-sized doses could compete with your medication and slightly throw off how well it controls your menopausal symptoms, things like hot flashes, night sweats, or vaginal dryness.

I want to be honest, though: this is only based on test-tube studies, not real people, so we don't truly know if it matters in the body. I wouldn't lose sleep over it, but before adding a concentrated skullcap supplement, run it by your pharmacist or doctor first.

Theoretically, Baikal skullcap might increase levels of drugs transported by P-glycoprotein.
In vitro and animal research suggests that baicalein, oroxylin A, and wogonin, constituents of Baikal skullcap, can inhibit P-glycoprotein. This effect has not been reported in humans.

Here's the simple version. Your hormone therapy (Prempro) contains estrogen plus a progestin, and some of these hormone molecules are moved around your body by a little cellular pump called P-glycoprotein. In lab dishes and animal studies, certain compounds in Baikal skullcap (like baicalein and wogonin) seem to slow that pump down. If that happened in people, slightly more hormone could stay in your system, which could nudge up the usual estrogen side effects such as breast tenderness, bloating, nausea, or spotting.

The honest truth is this is theoretical only. It has never actually been shown in humans, and it's rated a minor concern. I wouldn't lose sleep over it, but since this is a hormone product, it's worth a quick chat with your pharmacist or doctor before adding the herb.
