Fedratinib Hydrochloride falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might increase levels of drugs metabolized by CYP2C19 enzymes.
In vitro evidence suggest that wogonin, a constituent of Baikal skullcap, modestly inhibits the activity of CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist for fedratinib (Inrebic). Your body partly relies on a liver enzyme called CYP2C19 to help clear this medicine. One compound in Baikal skullcap, wogonin, was shown in lab dish studies to mildly slow that enzyme down. If that happened in a person, fedratinib could build up a bit and you might notice more of its usual side effects, like diarrhea, nausea, vomiting, tiredness, or muscle aches. Fedratinib works on its own, so this is not a prodrug situation.

The reassuring part: this is only a theoretical, test-tube finding and has never been confirmed in actual patients. Still, because fedratinib is a serious cancer drug with careful dosing, please check with your oncology team or pharmacist before adding this herb.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
