Baikal Skullcap and Glecaprevir, Pibrentasvir

Here is the interaction we found between these two, with the evidence behind it.

Minor

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Glecaprevir, Pibrentasvir

Glecaprevir, Pibrentasvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might increase levels of drugs transported by P-glycoprotein.
In vitro and animal research suggests that baicalein, oroxylin A, and wogonin, constituents of Baikal skullcap, can inhibit P-glycoprotein. This effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Mavyret (glecaprevir and pibrentasvir). Your body uses a tiny pump called P-glycoprotein to move this medication around and help clear it. In test tube and animal studies, some compounds in Baikal skullcap (like baicalein and wogonin) appear to slow that pump down. If that happened in people, more of the drug could linger in your body, which could nudge up side effects like headache, tiredness, or nausea.

Now the honest part: this is only a theoretical concern. It has not been shown in humans, and it's rated as a minor interaction. I wouldn't lose sleep over it, but since you're treating hepatitis C and want this therapy to work cleanly, please run any skullcap supplement by your pharmacist or doctor first.

From our pharmacist’s overview on the Glecaprevir, Pibrentasvir page

“Mavyret (glecaprevir and pibrentasvir) is a short course of pills meant to cure hepatitis C, so I read its supplement list with real caution. Nearly everything on it can shift how much of the drug reaches your blood, mostly through the CYP3A4 enzyme or the P-glycoprotein transporter that handles it.”
