Methenamine Mandelate falls under ANTITHYROID DRUGS — the group Baikal Skullcap interacts with.

Theoretically, concomitant use of Baikal skullcap and antithyroid drugs may result in additive activity and increase the risk of hypothyroidism.
In an animal hyperthyroid model, Baikal skullcap improved levels of triiodothyronine (T3), thyroxine (T4), and thyroid stimulating hormone (TSH). The clinical significance of this effect is unclear.

Here's the gist: methimazole works by lowering how much thyroid hormone your gland makes. In a single animal study, Baikal skullcap also seemed to nudge thyroid numbers (T3, T4, and TSH) in a similar direction. So in theory, taking the two together might add up and push your thyroid activity down a little more than expected. That could show up as feeling unusually tired, cold, sluggish, constipated, or noticing weight gain, basically signs your thyroid is running too low.

I want to be honest though, this is based on lab and animal work only, not real patients, so we don't truly know if it matters in people. I wouldn't lose sleep over it, but since your thyroid dose is something we watch closely, please check with your doctor or pharmacist before adding this herb.

Methimazole tablets are used in adults and children with hyperthyroidism (an overactive thyroid) due to Graves' disease or toxic multinodular goiter when surgery or radioactive iodine is not the right option. They work to lower thyroid hormone levels and ease symptoms.

Methimazole tablets are also used to bring thyroid hormone levels down before a patient undergoes thyroidectomy (thyroid surgery) or radioactive iodine treatment, making those procedures safer.
