Here's the gist for Adempas (riociguat). This medicine works by relaxing and widening blood vessels, which is great for the lungs but can also drop your overall blood pressure. Baikal skullcap has shown some blood-pressure-lowering activity in animal studies (mainly from a compound called baicalein), so the concern is that taking the two together could add up and push your pressure too low.

If that happened, you might feel dizzy, lightheaded, faint, or unusually tired, especially when standing up. I want to be honest though: this is a theoretical worry based only on lab and animal research, not real human studies, so it's far from certain. Since riociguat already needs careful dose adjustments, I'd run any skullcap use by your pharmacist or doctor first rather than starting it on your own.

Theoretically, Baikal skullcap might increase levels of drugs transported by P-glycoprotein.
In vitro and animal research suggests that baicalein, oroxylin A, and wogonin, constituents of Baikal skullcap, can inhibit P-glycoprotein. This effect has not been reported in humans.

Here's the gist for Adempas (riociguat): this drug is moved around the body in part by a transporter pump called P-glycoprotein. Lab and animal studies hint that compounds in Baikal skullcap (baicalein, wogonin, and others) can slow that pump down. If that happened in people, a bit more riociguat could stay in your system, which in theory might nudge up its blood-pressure-lowering effect. With riociguat, that could mean things like dizziness, lightheadedness, headache, or feeling faint when you stand up.

I want to be honest, though: this is theoretical and has only been seen in test tubes and animals, not in humans, so it's rated minor. Still, since riociguat strongly affects blood pressure, please run any skullcap product by your pharmacist or doctor first.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
