Here's the simple version. Your inhaler, aclidinium (Tudorza Pressair), works by blocking acetylcholine in your airways to help them open up. Belladonna does the same kind of blocking, just throughout the body. So stacking the two means more anticholinergic effect overall, and that's where trouble can creep in.

What you might notice is more dry mouth, blurry vision, constipation, trouble urinating, a faster heartbeat, or feeling flushed and warm. This concern is theoretical, based on how both work rather than firm studies, and aclidinium mostly stays in the lungs. Still, belladonna is genuinely risky on its own, so please don't combine them without talking to your pharmacist or doctor first.
