Why this applies to Irinotecan Hydrochloride

Irinotecan Hydrochloride falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Black Tea interacts with.

Theoretically, black tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.
In vitro, black tea extract inhibits organic anion-transporting polypeptide (OATP)2B1. OATP2B1 is expressed in the small intestine and liver and is responsible for the uptake of drugs and other compounds. In an animal model, black tea extract was found to inhibit the absorption of rosuvastatin, a substrate of OATP2B1. However, this effect has not been reported in humans.

Here's the gist: black tea contains polyphenols that, in lab dishes, can block a transporter called OATP2B1 that helps move certain drugs from your gut into your body. In theory, that could lower how much of an OATP-handled drug gets absorbed. The catch is that irinotecan (Onivyde) is given as an IV infusion, not a pill, so a gut-absorption effect like this really doesn't apply the same way.

On top of that, this is very weak evidence, just test-tube and animal data with a different drug (rosuvastatin), and nothing seen in real patients. So I wouldn't lose sleep over a cup of tea here. That said, anything affecting a chemo drug deserves a quick check, so please run your full medication and supplement list past your oncology team or pharmacist.
