Butabarbital, Phenobarbital, Secobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection worth knowing. Barbiturates like phenobarbital, butabarbital, and secobarbital are well-known to speed up a liver enzyme called CYP3A4. Normally that just means your body clears certain substances faster. But borage is the wrinkle here. The leaves, flowers, and seeds of the borage plant can contain natural compounds called pyrrolizidine alkaloids (PAs), and CYP3A4 turns those PAs into more toxic byproducts that can stress your liver.
So in theory, taking a barbiturate could ramp up that enzyme and create more of these liver-harming byproducts from borage. The good news: this is based on lab and animal data only, not strong human evidence, and properly made borage seed oil labeled 'PA-free' contains little to none of these alkaloids. Please choose a certified PA-free product and check with your pharmacist before combining them.
