Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the thing to keep in mind with this pairing. Ivosidenib (Tibsovo) is one of those medicines that can speed up a liver enzyme called CYP3A4. That matters because some parts of the borage plant (the leaf, flower, and seed) contain natural compounds called pyrrolizidine alkaloids, or PAs, which can be hard on the liver. CYP3A4 is what turns those PAs into their more toxic form. So in theory, if ivosidenib revs up that enzyme, your body could make more of the liver-damaging byproducts from borage.

I want to be honest that this is a theoretical concern based on lab work, not real-world cases. Borage seed oil usually has little to no PAs, so look for products clearly labeled 'PA-free.' Please run any borage product by your oncology team first, since ivosidenib already needs careful monitoring.

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
