Why this applies to Encorafenib

Encorafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the situation with breadfruit and your encorafenib (Braftovi). Your body clears encorafenib using a liver enzyme called CYP3A4. Lab and animal studies hint that a compound in breadfruit leaves may slow that enzyme down a little. If that happened in a real person, encorafenib could build up more than expected, which might mean stronger side effects like fatigue, nausea, joint pain, skin changes, or in rare cases heart rhythm concerns.

The good news is this is a theoretical concern only, based on test-tube and animal work, and it was rated minor. Eating breadfruit as a food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger question mark, so since encorafenib is a serious cancer drug, please run any breadfruit supplement past your oncology team or pharmacist first.

About Encorafenib

What Is Encorafenib Used For?

Brand name: Braftovi

Braftovi is used to treat cancers that have a specific change (mutation) in a gene called BRAF — specifically the V600E or V600K mutation, which must be confirmed by an FDA-authorized test before starting treatment. It is always used in combination with other medicines, never alone as standard therapy.

Braftovi (in combination with binimetinib) treats unresectable or metastatic melanoma with a BRAF V600E or V600K mutation. For metastatic colorectal cancer (mCRC) with a BRAF V600E mutation, Braftovi is used either with cetuximab and fluorouracil-based chemotherapy, or with cetuximab alone after prior therapy. Braftovi (in combination with binimetinib) also treats metastatic non-small cell lung cancer (NSCLC) with a BRAF V600E mutation.
