These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Brussels Sprout interacts with whole categories of medication. Acetaminophen, Butalbital, Caffeine falls into 3 of those categories, so it can interact with Brussels Sprout in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Butalbital, Caffeine
Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Brussels Sprout interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the part of your headache pill that matters here: the caffeine. Caffeine is processed by a liver enzyme called CYP1A2, and lab and animal studies hint that Brussels sprouts may speed that enzyme up a bit. If that happened in people, your body might clear caffeine a little faster, so you could notice slightly less of caffeine's usual kick (less of that mild alertness or headache-helping boost). The acetaminophen and butalbital parts aren't really affected by this.
Honestly, this is theoretical and based only on animal data, so I wouldn't lose sleep over it. Eating Brussels sprouts as a normal vegetable is very unlikely to change how this medicine works. If you ever feel it's not helping your tension headaches, just check in with your pharmacist or doctor.

GLUCURONIDATED DRUGS
2 of 3Acetaminophen, Butalbital, Caffeine also falls under GLUCURONIDATED DRUGS.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for your headache medicine. The acetaminophen (Tylenol) part of this combo is partly cleared by a process called glucuronidation, where your liver attaches a tag to the drug so you can flush it out. Some research suggests that regularly eating cabbage and Brussels sprouts can speed up that tagging process, which could lower acetaminophen levels a little and make the pain relief feel slightly weaker.
Honestly, this is a mild and somewhat theoretical concern. Normal amounts of Brussels sprouts at dinner are very unlikely to throw off your headache relief. I'd only pay closer attention if you're eating large amounts daily or taking concentrated supplements. If your headaches stop responding the way they used to, mention it to your pharmacist or doctor rather than adjusting anything on your own.

ACETAMINOPHEN (Tylenol, others)
3 of 3Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's what's going on with your headache medicine. The acetaminophen part of this combo (the same active ingredient as Tylenol) gets cleared by the liver through a process called glucuronide conjugation. Eating Brussels sprouts regularly seems to speed that cleanup pathway up a bit, so your body removes the acetaminophen a little faster. In one clinical study, a daily cabbage and Brussels sprout diet lowered acetaminophen levels by up to about 16 percent.
In plain terms, a steady, sprout-heavy diet could make the acetaminophen portion work slightly less for pain. That's a fairly small change, and an occasional serving at dinner is unlikely to matter. This doesn't change the butalbital or caffeine. If your tension headaches aren't easing as well as before, mention it to your pharmacist or doctor rather than adjusting anything yourself.

From our pharmacist’s overview on the Acetaminophen, Butalbital, Caffeine page
“Alcohol is the interaction I would flag first for anyone taking acetaminophen, butalbital, and caffeine (Esgic, Fiogesic, and others). Drinking raises the risk of liver injury from the acetaminophen component, and 38 of the 252 supplement ingredients on file carry that same liver concern.”

About Brussels Sprout
What is Brussels Sprout?
- General nutrition and fiber
- Antioxidant support
- Digestive health
- Heart health support
- Cancer-prevention interest
Brussels sprouts are a nutritious cruciferous vegetable rich in fiber, vitamin C, vitamin K, and plant compounds called glucosinolates. Eating them as part of a balanced diet is healthy and safe for most people, but supplement-style claims (like preventing cancer) are not well proven. If you take blood thinners or have thyroid concerns, talk with your pharmacist or doctor about your intake.

What Is Acetaminophen, Butalbital, and Caffeine Used For?
Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet
This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).
All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
