BISPHOSPHONATES
Why this applies to Alendronate Sodium
Alendronate Sodium falls under BISPHOSPHONATES — the group Calcium interacts with.
Advise patients to take bisphosphonates at least 30 minutes before calcium, but preferably at a different time of day. Calcium supplements decrease absorption of bisphosphonates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the practical side of this one. Your body only absorbs a tiny fraction of alendronate to begin with, and calcium makes that even worse. When the two meet in your stomach, calcium binds to the alendronate and blocks it from getting into your bloodstream. The result isn't a dangerous reaction, it's a wasted dose, meaning less protection for your bones over time.
The fix is simple timing. Take your alendronate first thing in the morning on an empty stomach with plain water, then wait at least 30 minutes (ideally longer) before any calcium supplement, food, or other pills. This is well-accepted advice, so don't stop either one. Just space them out, and check with your pharmacist if your schedule makes that tricky.

About Alendronate
What Is Alendronate Used For?
Alendronate slows bone breakdown. It is used mainly for osteoporosis (thin, fragile bones that break more easily).
Uses differ by product. Alendronate tablets and Fosamax tablets are used for osteoporosis in postmenopausal women (treatment and prevention) and in men. They are also used for glucocorticoid-induced osteoporosis and Paget's disease of bone. Alendronate sodium oral solution and Binosto effervescent tablets are used only to treat osteoporosis in postmenopausal women and to increase bone mass in men with osteoporosis.
