Aspirin, Caffeine, Orphenadrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the piece worth knowing: this combo product contains caffeine, and your liver clears caffeine using an enzyme called CYP1A2. CBD can slow that enzyme down, so caffeine may hang around in your body longer than usual. This part actually has good human evidence behind it, not just lab guesses.
In plain terms, the caffeine portion may hit a little harder. You might notice things like jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired and anxious. None of this is dangerous for most people, but if you're sensitive to caffeine or already use other caffeine sources, it can add up. The aspirin and orphenadrine parts aren't the main concern here. Before mixing CBD with this medicine, check with your pharmacist or doctor.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combination pill bundles aspirin, caffeine, and a muscle relaxer called orphenadrine. CBD can slow down a liver enzyme (CYP3A4) that helps the body clear certain medicines. Orphenadrine relies in part on that pathway, so adding CBD could let it build up a bit higher than usual. In real life that might mean stronger drowsiness, dizziness, dry mouth, blurred vision, or a fast heartbeat. The caffeine in the product can also add to that jittery, racing feeling.
None of this means you can't use both, but it's worth a quick check. Talk with your pharmacist or doctor before pairing CBD with this medicine, especially if you feel more sleepy or off than normal.

Caffeine is a substrate of CYP1A2, and cannabidiol has been shown to inhibit CYP1A2 metabolism. A pharmacokinetic study in healthy adults shows that taking oral cannabidiol, starting at 250 mg once daily and titrating to 750 twice daily over a total of 24 days, increases the peak serum level of caffeine by 15% and the overall exposure to caffeine by 95% after a single dose of caffeine 200 mg taken on day 23. Other clinical research also shows that cannabidiol modestly increases the area under the curve of caffeine but does not increase peak serum levels of caffeine.

Here's the piece to pay attention to with this combo pill: the caffeine part. CBD slows down a liver enzyme (CYP1A2) that normally clears caffeine out of your system. In a good-quality study, regular CBD use roughly doubled the body's overall exposure to a dose of caffeine. So the caffeine in your Orphengesic or Norgesic tablet could linger longer and hit a bit harder.
In real life that might mean feeling jittery, a racing or pounding heartbeat, trouble sleeping, or an upset stomach. The aspirin and orphenadrine parts aren't really affected by this. If you use CBD regularly along with this medicine, it's worth a quick chat with your pharmacist or doctor about timing and how much caffeine you're getting overall.

CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES

In vitro research shows that cannabidiol inhibits CYP2B6. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2B6 substrates might increase the risk for adverse effects from these substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product pairs aspirin and caffeine with orphenadrine, a muscle relaxant. The piece that matters here is orphenadrine, which is partly broken down by a liver enzyme called CYP2B6. In lab (test tube) studies, CBD appears to slow that enzyme down. If that happened in real life, orphenadrine could build up a bit, which might mean more of its usual side effects, like drowsiness, dry mouth, blurred vision, dizziness, or a faster heartbeat.
I want to be honest about the strength of this, though. It's theoretical, based only on lab work, and hasn't actually been reported in people. So I wouldn't lose sleep over it, but if you use a CBD product regularly alongside this medicine, mention it to your pharmacist or doctor first so they can keep an eye on how you feel.
