Belladonna Alkaloids, Phenobarbital falls under CNS DEPRESSANTS — the group Cannabidiol (cbd) interacts with.
Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this combo: your product (Donnatal or Barbidonna) contains phenobarbital, a barbiturate that already slows down the central nervous system. CBD, especially at higher supplement doses, may add to that calming, sleepy effect. Stacked together, you could feel more drowsy, dizzy, foggy, or unsteady on your feet than you would expect, and that adds up if you're driving or older.
There's also a second wrinkle. Phenobarbital and CBD are both handled by the same liver enzymes, so levels can shift in either direction over time. This concern is mostly theoretical and based on how the drugs behave, not hard proof. Still, please loop in your pharmacist or doctor before pairing them so they can watch your dosing.

CYTOCHROME P450 2C19 (CYP2C19) INDUCERS

Cannabidiol is a substrate of CYP2C19 enzymes. Theoretically, drugs that induce CYP2C19 enzymes might decrease the levels and effects of cannabidiol.

Here the medicine is the one affecting your CBD, not the other way around. Phenobarbital (the active ingredient in Donnatal that pairs with the belladonna alkaloids) is a known liver enzyme booster. It speeds up CYP2C19, the same enzyme that helps break down CBD. So in theory, taking them together could push your body to clear CBD faster, leaving less of it around and possibly making your CBD feel weaker or work less well.
Keep in mind this is theoretical, based on how the drugs behave on paper rather than solid human studies. It also doesn't change how the phenobarbital or belladonna part works for you. If you rely on CBD for a real reason, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that induce CYP3A4 enzymes might reduce the levels and effects of cannabidiol.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Donnatal (which contains phenobarbital). Phenobarbital is a well-known booster of a liver enzyme called CYP3A4, the same enzyme your body uses to break down CBD. By revving up that enzyme, phenobarbital can speed up how fast CBD is cleared, which may leave you with lower CBD levels and a weaker effect than you expected. In short, your CBD may simply not work as well.
Good news: this concern is theoretical, based on how these drugs behave rather than on solid human studies. It doesn't change how your phenobarbital works. Still, phenobarbital is a sensitive medication, so before adding any CBD product, check in with your pharmacist or doctor first.

CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES
4 of 4Belladonna Alkaloids, Phenobarbital also falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES.
In vitro research shows that cannabidiol inhibits CYP2B6. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2B6 substrates might increase the risk for adverse effects from these substrates.

Here's the gist for products like Donnatal: the phenobarbital part is handled in part by a liver enzyme called CYP2B6. In lab dish studies, CBD appears to slow that enzyme down. If that played out in the body, phenobarbital could clear a little slower and build up, which might mean more drowsiness, dizziness, grogginess, or trouble concentrating. Phenobarbital is a barbiturate with a fairly narrow safety window, so even modest shifts deserve respect.
That said, this is only a theory based on test-tube work, not something actually seen in people. Still, since both CBD and phenobarbital can make you sleepy on their own, please loop in your pharmacist or doctor before combining them rather than changing anything yourself.
