Crizotinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the thing to know if you're pairing CBD with crizotinib (Xalkori). Crizotinib blocks a liver enzyme called CYP3A4, and CBD happens to be cleared by that same enzyme. So on paper, crizotinib could slow down how fast your body breaks down CBD, letting CBD levels drift higher than expected. That could mean more of CBD's own effects, like drowsiness, diarrhea, changes in appetite, or tiredness.

I want to be straight with you though: this is a theoretical concern based on how these two are processed, not something proven in real patients. It's worth a quick chat with your oncology team or pharmacist before adding CBD, since they're already watching closely while you're on cancer treatment. Please don't change anything on your own.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for crizotinib (Xalkori). Your liver uses an enzyme called CYP3A4 to break this cancer medicine down. CBD can slow that same enzyme, so crizotinib may not clear out as fast and its levels in your blood can creep up. Crizotinib is an active drug (not a prodrug), so more of it usually means stronger effects and a higher chance of side effects.

With crizotinib, that could look like more nausea, vomiting, diarrhea, vision changes, swelling, or tiredness, plus possible effects on your liver enzymes or heart rhythm (QT). Please don't change anything on your own. Loop in your oncology team or pharmacist before adding CBD so they can decide if it's safe or if monitoring is needed.

About Crizotinib

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) is used to treat adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for the ALK or ROS1 gene abnormality, as confirmed by an FDA-approved test.

Xalkori is also used to treat pediatric patients aged 1 year and older and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL). Additionally, it treats adult and pediatric patients aged 1 year and older with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Note: Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
