Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

Here's the simple version: both CBD and desipramine can make you feel drowsy or foggy, so taking them together may stack that sleepiness on top of each other. Desipramine (Norpramin) is a tricyclic antidepressant, and on its own it can cause drowsiness, dizziness, dry mouth, and a feeling of being a little slowed down. Adding CBD, especially at higher concentrated doses, could nudge those effects up a notch.

I'll be honest with you, this concern is mostly theoretical, based on how these compounds behave rather than strong studies in people. Still, it's worth a quick chat with your pharmacist or doctor before combining them, particularly if you drive or already feel sedated. Don't change your desipramine on your own.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to clear out desipramine. In lab dishes, CBD can slow that enzyme down. If that happened in real life, desipramine could build up to higher levels, which might mean stronger side effects like a dry mouth, constipation, dizziness, drowsiness, a faster heartbeat, or trouble urinating.

The good news is that this is mostly a theoretical concern. A human study using another CYP2D6 drug found CBD didn't actually block it. Still, desipramine is one of those medicines where blood levels matter, so I wouldn't add CBD on your own. Check with your pharmacist or doctor first, especially with concentrated CBD products.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Desipramine is an active medication on its own, not a prodrug, so when its breakdown slows, more of it can build up in your blood. That can mean a stronger effect and more of the side effects this tricyclic is known for, things like dry mouth, constipation, dizziness when you stand up, drowsiness, a fast or irregular heartbeat, and trouble urinating.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the gist for desipramine and CBD. Your body uses a liver enzyme called CYP1A2 (along with others) to help clear desipramine out of your system. CBD can slow that enzyme down, so the medicine may build up a bit higher than usual. Desipramine is a tricyclic antidepressant, which is an active drug (not a prodrug), so more of it in your blood usually means stronger effects and a greater chance of side effects like dry mouth, constipation, dizziness, a fast or irregular heartbeat, or feeling overly drowsy.

One thing to keep in mind: the studies showing this used fairly high CBD doses. Still, since tricyclics can be touchy, it's worth a quick chat with your pharmacist or doctor before combining them, rather than changing anything on your own.

What Is Desipramine Used For?

Brand names: Norpramin, Desipramine, Pertofrane

Desipramine hydrochloride tablets — sold under the brand names Norpramin, Desipramine HCL, and Desipramine Hydrochloride — are FDA-approved for the treatment of depression in adults. Norpramin tablets and the generic formulations share this same approved indication.

Desipramine tablets are not approved for use in children. Adolescents and elderly patients may be prescribed it in certain circumstances, but at lower doses and under close supervision.
