Why this applies to Dorzolamide, Timolol

Dorzolamide, Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the situation with your Cosopt eye drops (dorzolamide and timolol) and CBD. The timolol part is a beta blocker that the body partly clears using a liver enzyme called CYP2D6. In lab tests, CBD can slow that same enzyme down. The thinking is that if less timolol gets broken down, a bit more could end up in your system, which could mean slightly stronger beta blocker effects like a slower heart rate, lower blood pressure, tiredness, or feeling lightheaded.

That said, this is only a theoretical concern based on test-tube data, and a human study with a similar CYP2D6 drug showed no real effect. Eye drops also deliver a fairly small amount into the bloodstream. I wouldn't be alarmed, but it's worth a quick chat with your pharmacist or doctor, especially if you notice your pulse running low.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
