Why this applies to Hydrochlorothiazide, Pindolol

Hydrochlorothiazide, Pindolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Viskazide is a combo pill containing pindolol (a beta-blocker) and hydrochlorothiazide (a water pill). The piece that matters here is pindolol, which your body partly clears using a liver enzyme called CYP2D6. In lab studies, CBD can slow that enzyme down. In theory, that could let a bit more pindolol build up, which might nudge your heart rate or blood pressure lower than expected. Realistically that could show up as dizziness, tiredness, a slow pulse, or feeling lightheaded when you stand.

That said, this is theory-level only. It comes from test-tube research, and a human study found CBD did not meaningfully affect another CYP2D6 drug. So a real-world problem is far from certain. Still, I'd give your pharmacist or doctor a heads-up before starting CBD so they can keep an eye on your blood pressure and heart rate.
