Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the gist for losartan and CBD. Your liver uses an enzyme called CYP2C9 to process losartan, and CBD can slow that enzyme down. When that happens, a bit more losartan can hang around in your bloodstream than usual. With losartan, that often shows up as its blood-pressure-lowering effect getting stronger, so you might notice dizziness, lightheadedness when you stand up, or feeling unusually tired.

This isn't just a hunch. Human studies actually saw losartan levels rise when people also used CBD, so it's worth taking seriously. The good news is this is usually manageable. Please don't stop or adjust either one on your own. Just check in with your pharmacist or doctor, who may want to keep an eye on your blood pressure for a while.

Here's the wrinkle with losartan (Cozaar). It's actually a prodrug, meaning your body has to convert it into its active form to do most of its blood-pressure-lowering work. That conversion happens through liver enzymes including CYP3A4, the same enzyme that CBD can slow down. So in theory, adding CBD could mean less of the active drug is formed, which might make losartan work a little less well at controlling your blood pressure.

I want to be honest that this part is mostly based on how the enzyme behaves, not on direct losartan studies, so the real-world effect may be small. Still, if you take CBD regularly, it's worth keeping an eye on your blood pressure readings and checking in with your pharmacist or doctor rather than guessing.

Here's the gist: your body uses an enzyme called CYP2C9 to help process losartan. CBD slows that enzyme down, so losartan can hang around at higher levels in your blood. In one well-run study, a single large dose of CBD (640 mg) bumped up losartan exposure by about 77%, which is a meaningful jump.

What might you feel? Losartan lowers blood pressure, so more of it on board could push your pressure too low. That can mean dizziness, lightheadedness when you stand up, or feeling tired or washed out. It's worth noting that small, food-style amounts of hemp are unlikely to matter much, but concentrated CBD products are a different story. Please don't change anything on your own. Check in with your pharmacist or doctor first.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
