Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the situation with these two together. Your liver uses an enzyme called CYP2C9 to clear piroxicam out of your system. CBD can slow that enzyme down, so piroxicam may not break down as quickly and can build up to higher levels than usual. Piroxicam already has a long stay in the body, so this matters.

In plain terms, you could see more of the typical side effects from piroxicam, like stomach upset, heartburn, an increased chance of stomach irritation or bleeding, fluid retention, or kidney strain. This effect is rated moderate and likely, backed by solid human studies on other CYP2C9 drugs. Don't change anything on your own. Just check with your pharmacist or doctor first, especially if you take the oral capsules daily.
