Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist: the valsartan part of Entresto is partly cleared by a liver enzyme called CYP2C9, and CBD tends to slow that enzyme down. When the enzyme works more slowly, valsartan can hang around in your body a bit longer than usual. That means a little more blood pressure lowering than you'd expect. We've actually seen this play out in studies where CBD raised levels of losartan, a close cousin of valsartan, so this isn't just theory.
In real life, that could show up as dizziness, lightheadedness when standing up, unusual tiredness, or feeling faint. Please don't stop or change anything on your own. Just check in with your pharmacist or doctor so they can keep an eye on your blood pressure and decide if any adjustment is needed.

“Entresto (sacubitril and valsartan) is a heart failure medicine, and heart failure patients tend to be heavy supplement users, reaching for fish oil, potassium, and herbal blood-pressure remedies. That instinct carries real risk here.”

About Cannabidiol (cbd)
What is Cannabidiol (cbd)?
- Anxiety and stress
- Sleep problems
- Chronic pain
- Certain seizure disorders
- Inflammation
CBD is a compound from the cannabis (hemp) plant that does not cause a 'high.' It has strong evidence only for certain rare seizure disorders (as a prescription drug), while most other popular uses are still backed by limited or early research. Product quality varies a lot, so talk with your pharmacist or doctor before using it, especially if you take other medicines.
About Valsartan and Sacubitril
What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).
It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
