Why this applies to Vilazodone

Vilazodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the gist: your body uses an enzyme called CYP2D6 to help clear vilazodone (Viibryd). In lab dish studies, CBD has slowed that same enzyme down. If that happened in real life, vilazodone could build up a bit, which might mean more of its usual side effects like nausea, diarrhea, headache, trouble sleeping, or feeling jittery.

I want to be honest about how shaky this is, though. The concern comes only from test-tube research. When CBD was actually tested in people against another CYP2D6 drug, it did not block the enzyme. Vilazodone isn't a prodrug, so this isn't about losing its benefit. Still, if you want to add CBD, run it by your pharmacist or doctor first, especially since CBD products vary so much in strength.

Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the gist for Viibryd (vilazodone): your liver uses an enzyme called CYP2C19 to help clear this antidepressant. CBD slows that enzyme down, so vilazodone can build up a bit higher in your system than usual. This isn't a prodrug situation, so more drug simply means a stronger effect and a greater chance of side effects.

With vilazodone, that could show up as more nausea, diarrhea, dizziness, trouble sleeping, or headache. The evidence that CBD blocks CYP2C19 is solid, so this is a real and likely interaction, though everyone reacts differently. Please don't change anything on your own. Check with your pharmacist or doctor first, and let them know how much CBD you're using, since concentrated products matter more than tiny amounts.

Here's the gist: your body uses a liver enzyme called CYP3A4 to clear vilazodone (Viibryd) out of your system. CBD can slow that enzyme down. When that happens, vilazodone tends to hang around longer and its levels can creep up, which may mean stronger effects and a better chance of side effects like nausea, diarrhea, headache, trouble sleeping, or feeling jittery.

Vilazodone is not a prodrug, so higher levels mean a stronger punch, not a weaker one. We have solid human evidence that CBD raises levels of similar antidepressants. This isn't a reason to panic, but please loop in your pharmacist or doctor before combining them so they can watch how you feel and adjust if needed.

MDD is more than just feeling sad; it's a clinical condition involving persistent low mood, loss of interest, and other symptoms that significantly affect daily life.

Vilazodone is not approved for use in children or teenagers. Its safety and effectiveness have not been established in pediatric patients.
