Vonoprazan, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabidiol (cbd) interacts with.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing worth knowing if you're taking the Voquezna Triple Pak (vonoprazan, amoxicillin, and clarithromycin) for an H. pylori infection. One of those three, clarithromycin, is a fairly strong blocker of a liver enzyme called CYP3A4. That same enzyme helps break down CBD. So in theory, while you're on this combo, your body may clear CBD a bit slower, letting CBD levels build up.
What that could mean in real life is stronger CBD-type effects, things like drowsiness, lightheadedness, dry mouth, or an upset stomach. I'll be honest with you though: this is a theoretical concern based on how the drugs work, not on solid studies, so don't panic. The H. pylori course is also short, usually about two weeks. Still, it's worth mentioning your CBD to your pharmacist or doctor before you start, just so they can keep an eye on how you feel.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Vonoprazan, Amoxicillin, Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Voquezna Triple Pak combines three medicines used together for about two weeks to treat an H. pylori stomach infection. The piece that matters here is clarithromycin, which the body partly clears using a liver enzyme called CYP3A4. CBD can slow that enzyme down, so clarithromycin (and to a smaller degree the others) may build up a bit higher than usual in your blood.
In real terms, that could mean more of clarithromycin's typical complaints, like stomach upset, nausea, a metallic taste, or diarrhea. Because this is a short course of treatment, the risk is limited, but it is still worth a quick chat with your pharmacist or doctor before mixing in CBD. Please don't stop or change any of these medicines on your own.
