Zileuton falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the gist for Zileuton (Zyflo): your liver uses an enzyme called CYP1A2 to help clear it out of your system. CBD can act like a brake on that enzyme. When that pathway slows down, Zileuton can hang around longer and build up a bit, which may nudge its effects and side effects upward.

For Zileuton specifically, that could mean more of its known issues, like upset stomach, headache, or in rarer cases stress on the liver (this drug already needs liver monitoring). This isn't a prodrug situation, so the effect goes up, not down. It's a moderate, likely interaction, but everyday foods aren't the concern here, concentrated CBD products are. Before mixing them, check in with your pharmacist or doctor so they can keep an eye on your liver labs.

Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the gist if you're using CBD alongside Zileuton (Zyflo). Zileuton can act as a mild blocker of a liver enzyme called CYP3A4, which is one of the pathways your body uses to break down CBD. When that pathway is slowed down, CBD may clear out more slowly and build up a bit, which could make its effects (and side effects like drowsiness, an upset stomach, diarrhea, or changes in appetite) more noticeable.

I'll be honest, this is a theoretical concern based on how these compounds are processed, not something proven in real patients, so I don't want you to lose sleep over it. Zileuton itself isn't a prodrug, so it still works the way it should. If you plan to combine them, just loop in your pharmacist or doctor first so they can keep an eye on how you feel.

In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the simple version: your body uses a liver enzyme called CYP2C9 to help clear Zileuton out. CBD can slow that enzyme down, so Zileuton may build up a bit more than usual. Zileuton is the active medicine itself (not a prodrug that needs switching on), so higher levels mean a stronger drug effect and a higher chance of side effects.

With Zileuton, that could mean things like nausea, stomach upset, headache, or extra strain on the liver, which is already something this medicine is watched for. Here's the gist: your asthma medicine Zileuton is broken down in part by a liver enzyme called CYP3A4. CBD can slow that enzyme down. When the cleanup crew works slower, more Zileuton can hang around in your blood than usual, which may nudge up its effects and side effects.

With Zileuton, that could look like stomach upset, nausea, headache, or muscle aches, and the bigger thing to watch is your liver, since this drug can affect liver enzymes on its own. What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
