The product you're asking about pairs aspirin (a blood thinner that keeps platelets from clumping) with caffeine. Here's the thing to know: lab studies suggest cilantro can also make platelets a little less sticky. So in theory, taking concentrated cilantro on top of aspirin could add to that effect and nudge your bleeding risk up a bit, think easier bruising, nosebleeds, or stomach upset that lingers.
Now, keep this in perspective. This is theoretical, based only on test-tube work, not real patient cases. Eating cilantro as a garnish or in your salsa is very unlikely to cause trouble. The caution really applies to concentrated cilantro supplements or essential oil. If you use those regularly, just run it by your pharmacist or doctor first.
