Why this applies to Cablivi
Cablivi falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Cinchona interacts with.
In vitro evidence shows that the alkaloids quinine, quinidine, and cinchonine, constituents of cinchona, inhibit platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing about Cablivi (caplacizumab): it already works by reducing your body's ability to form clots, which is exactly why it helps in aTTP. It is not a prodrug that needs to be switched on, so you don't have to worry about cinchona blocking some enzyme. The concern is simpler than that: both work in the same direction.
In lab studies, the alkaloids in cinchona bark (quinine, quinidine, cinchonine) can keep platelets from clumping together. Stack that on top of Cablivi and, in theory, you could bleed more easily, think easy bruising, nosebleeds, bleeding gums, or harder-to-stop cuts. This is theoretical and based on test-tube data, not real patient reports, so don't panic. Still, given that bleeding is already Cablivi's main risk, please run cinchona by your doctor or pharmacist first.

Caplacizumab-yhdp injection is used to treat acquired thrombotic thrombocytopenic purpura (aTTP; a disorder in which the body attacks itself and causes clots, low amounts of platelets and red blood cells, and could cause other serious complications) in combination with plasma exchange therapy and immunosuppressant medications. Caplacizumab-yhdp is in a class of medications called antithrombotic agents. It works by blocking the action of a certain substance in the body that causes the symptoms of aTTP.
