Clomipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cinchona interacts with.

Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.
Quinidine, a constituent of cinchona, inhibits CYP2D6-mediated metabolism in humans.

Here's the gist for Anafranil (clomipramine). Your liver uses an enzyme called CYP2D6 to help clear this medicine out of your body. Cinchona bark contains quinidine, which can block that same enzyme. If the enzyme is slowed down, clomipramine can build up higher than expected in your system.

Clomipramine isn't a prodrug, so more drug hanging around means stronger effects and more side effects, things like a dry mouth, constipation, drowsiness, dizziness, a racing or irregular heartbeat, and trouble urinating. Right now this is a theoretical concern based on how the chemicals behave, not solid human studies with these two together. Still, since heart rhythm is a real worry with both, please check with your pharmacist or doctor before combining them.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
