Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.
Quinidine, a constituent of cinchona, inhibits CYP2D6-mediated metabolism in humans.

Here's the thing to know about desvenlafaxine (Pristiq): it's an active drug, not a prodrug, so it works as-is without needing to be switched on by your liver. Cinchona contains quinidine, which can slow down a liver enzyme called CYP2D6 that helps clear desvenlafaxine. If that enzyme is blocked, the medicine may build up a bit higher than expected.

What might you notice? Possibly more of the usual SNRI effects, like nausea, headache, jitteriness, trouble sleeping, raised blood pressure, or a faster heartbeat. The good news is that this is mostly theoretical, based on how the drugs behave on paper rather than real patient reports, and CYP2D6 plays only a smaller role in clearing desvenlafaxine. Still, please check with your pharmacist or doctor before combining them.

Here's the concern with pairing cinchona bark and your Pristiq (desvenlafaxine). Cinchona naturally contains quinine and quinidine, two compounds that can stretch out part of your heart's electrical cycle (called the QT interval). Desvenlafaxine, like other antidepressants in its family, can have a small effect on that same rhythm. Stack them together and the worry is an additive nudge that could, in rare cases, trigger an irregular heartbeat.

I want to be honest about the evidence here: this is theoretical, based on how these compounds behave, not on a pile of real-world reports. For most people the risk is low. Still, because cinchona supplements are unstandardized and unpredictable, please run it by your pharmacist or doctor before combining the two.

Desvenlafaxine extended-release tablets — sold under brand names including Pristiq, Desvenlafaxine ER, Desvenlafaxine Succinate, and others — are approved for the treatment of major depressive disorder (MDD) in adults.

This medication is not approved for use in children or adolescents. Its effectiveness was studied in clinical trials of adult outpatients who met standard diagnostic criteria for MDD.
