Ibuprofen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cinchona interacts with.

Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.
Quinidine, a constituent of cinchona, inhibits CYP2D6-mediated metabolism in humans.

Here's the wrinkle with this combo product: the hydrocodone part is a prodrug. Your liver uses an enzyme called CYP2D6 to convert hydrocodone into hydromorphone, which is the form that does most of the pain-relieving work. Cinchona bark contains quinidine, a known blocker of that same enzyme. So in theory, cinchona could slow that conversion and leave you with less pain relief than expected, even though the medicine is still in your system.

I'll be honest, this is a theoretical concern based on how the drugs behave, not something proven in real patients. The ibuprofen side isn't really affected. Cinchona itself can be risky on its own, so before mixing them, please check with your pharmacist or doctor.

Theoretically, taking cinchona might increase the drug effects and risk of bleeding with anticoagulant and antiplatelet drugs.
In vitro evidence shows that the alkaloids quinine, quinidine, and cinchonine, constituents of cinchona, inhibit platelet aggregation.

Here's the thing to keep in mind with this combination product. Ibuprofen is an NSAID, and NSAIDs already have their own effect on platelets, meaning they make it a little harder for your blood to clot. Cinchona bark contains quinine and related alkaloids that, at least in lab studies, also slow platelets down. Stack those two together and the worry is more bleeding or bruising than usual, things like nosebleeds, easy bruising, or stomach irritation that bleeds.

I'll be honest that this is a theoretical concern based mostly on test-tube data, not strong human evidence, so don't panic. The hydrocodone half isn't really the issue here. Still, since cinchona supplements are unpredictable, I'd run this past your pharmacist or doctor before combining them rather than guessing.
